Please fill out this waiver if you are participating in a Worcester Railers On Ice activation. All participants must have a ticket to the game and fill out the RSVP form in order to participate. If applicable, your Railers HC representative will provide more details regarding check in details. 

If the participant is under the age of 18, a parent/guardian must fill out this waiver on their behalf. All participants over the age of 18 must complete their own waiver.
